4. The person who is claiming the incentive for eye donation must be a
close relative of eye donor.
5. Transplantation of eye should
be taken place.

The application shall be made 30 days from the date of death

disabled person.

Should not be in receipt of Transport Allowance for
conveyance from the
Department or from any other source.

He / she should be in gainfull Employee / Pursue higher
study.

be submitted.
